Anyone associated with the ProMedica Health System and his or her immediate family members may have an account at ProMedica Federal Credit Union. Immediate family members include sons, daughters, mothers, fathers, and spouses. Relatives must clear the Chexsystems database to become a member. PHS employees wishing to open a checking account must clear the Chexsystems database.
Two or more people may be joint owners of any credit union account. All parties must sign the application.
To add someone else to your account, just complete the Account Change Form(PDF) or call your Member Services Representative at 479-4040. When adding a joint owner, a copy of his/her driver’s license must be included along with the original signature. You may fax or mail the form to the credit union.
Lifetime Account
Even if you leave the ProMedica system, you and your relatives can keep your accounts at the credit union for life. Once you become a member, your account is for life.

Complete the New Member Application

Maintain a $25 Minimum Balance
All members must maintain a $25 minimum balance in a savings account to establish and continue your shareholder status with the credit union, a member-owned cooperative. This is not a fee. It always remains your money. You can make this first deposit through payroll deduction or include it with your application.

This includes primary and joint applicants, co-borrowers, and authorized users on new and current share accounts, certificates, credit cards and loans.
