Automatic Payment change form (PDF) . We will help you complete this form and send it to any organization you want to authorize to make automatic withdrawals from your credit union account. The form will include the credit union’s routing number and a space for your ten-digit account number. Some companies may ask you to include a voided check with your authorization form.
